The HSA only applies to those enrolled with the Base Medical Plan
What is a Healthcare Savings Account?
- An HSA allows you to pay for eligible medical expenses on a pre-tax basis AND save for your future.
- nGenera will contribute $750 annually
- Deposits to your HSA will be made on a monthly basis - $62.50 per month
- Employees are also encourage to contribute through payroll deductions - Pre Tax

Maximum annual employee contribution to HSA for 2008:
- 2,900 if you have individual coverage
- 5,800 if you have family coverage
A Limited Flexible Spending Account may still be elected to pay for vision, dental, and all other eligible expenses with pre-tax funds.
If you already have a Principal HSA account, you do not need to re-enroll.

HSA Payroll Auth Form.doc- To change your payroll-deducted contribution, complete this form and send it to Amy Ross aross@ngenera.com or via fax to 832-202-0976.
3 Principal HSA Authorized Signer Form.pdf- If you want to authorize a new person to use the HSA funds, complete this form and forward it to Principal Bank.
5 HSA Rollover FOrm.pdf- If you want to roll funds from another HSA account into your nGenera/Principal HSA account, complete this form and send it to Principal Bank along with a check for the rollover funds.
