Basic Life and AD&D Insurance
- 3x your base annual salary to a maximum of $500,000
Short Term Disability
- The plan pays you 60% of your base salary, to a maximum of $2,500 per week
Long Term Disability
- The plan pays you 60% of your base salary, up to $15,000 per month
Optional Life / AD&D Coverage
- Employees may purchase addition al coverage in $10,000 increments up to 5x your base annual salary of $300,000, whichever is smaller
- Depend Coverage Available - Souse coverage in 5,000 increments up to 2.5x employee's base annual earnings not to exceed 50% of the employee coverage election and Child $10,000 (age 6 months to 19 years)
- Evidence of Insurability may be required
- Supplemental dependent Life / AD& D can only be purchased WITH employee coverage
